ABSTRACT

Regarding the worldwide efforts to implement and improve mother friendly services, and the rights of mothers for receiving quality care, there is a need for effective leadership and management of these hospital resources. Research in health sectors has shown that managers' leadership style is related to effectiveness, efficiency, productivity. The purpose of this study, was to examine the relationship between leadership style and implementation process of the "mother friendly Hospital Initiative" in selected hospitals in the West Country [province of Kurdistan, Hamedan and Kermanshah] in 1391. The study had a descriptive, correlational design. leadership style was measured by two questionnaires using a 5 point Likert scale, and a checklist for assessing the implementation of mother friendly services. The questionnaire were completed by managers and hospital personnel. The sample of the study consisted of 37 managers and 333 employees in selected hospitals in the provinces of Kurdistan, Hamedan and Kermanshah participated in this study. Initiatives process of the mother friendly hospital was evaluated by the monitoring list of mother friendly hospital in ministry of health. Validity of the questionnaire was assessed by content validity method and, the reability of the questionnaires was determined by internal consistency [r=0.798] for managers, [r=0.89] for staff, and the reability of the monitoring check list of mother friendly hospital was assured by inter-rater method [rS0/885]. Data were analyzed done by Chi-square, Fisher Exact Test, Pearson correlation and one way anova tests using the SPSS software [version 16]. The results showed that leadership style of managers' views are as follows: 67.6% Management consulting style, 24.3% benevolent- Autocratic style and the leadership style of 8.1% was Participative. While 61% from the employee's perspective, leadership styles used by managers, was benevolent-Autocratic, 21.3% consulting, 14.4% Autocratic - exploitation, 3.3% participative. It can be said that the results were Relatively satisfactory in about the process of implementing measures in mother friendly hospitals and There is significant relationship between the leadership style employed in the first third. Fifth, eighth and ninth proceedings in views of managers and employees from the perspective of the second, fourth and seventh steps, based on ANOVA. So th, the average ratio was higher in e authoritarian leadership style compared with and collaborative consulting style The findings of this study showed that autocratic leadership style was positively correlated to more accelerated implementation of mother friendly hospital practices

ABSTRACT

Rehabilitation interventions have significant role in reducing the disabilities of Down syndrome. Due to great role of balance in the function of individuals particularly in Activity of Daily Living and the effect of strength on balance, the aim of this study was to investigate the effects of Progressive Resistive Exercise on functional balance in children with Down syndrome. Fifteen girls children aged between 8 and 12 years, with Down syndrome participated in this study by available sampling method. The subjects participated three times per week in a six-week progressive strengthening program. During intervention, 3 of them excluded. Balance were measured by Berg Balance Scale and right/left hip abductor and flexor, knee extensor and flexor muscles isometric strengths were measured by Nickolas hand-held dynamometer. Paired-sampled T-Test was used for data analysis. The results of this study showed that the mean of functional balance and muscles isometric strengths were significantly increased after intervention [P<0.001]. It seems that the results of this study confirm the effect of progressive resistance training on progressing functional balance, and 8-12 years old children with Down syndrome can benefit from these exercises

ABSTRACT

Gastric cancer [GC] is one the most common causes of death worldwide. Despite the decreasing rate for GC in the world during recent years, it remains the most common cancer in our country, especially among men. This study aimed to assess the survival of the patients with GC and to determine the risk factors involved. A total of 400 patients with GC in the Cancer Institute of Imam Khomeini Hospital, were entered in the study during 2003 - 2007. Patients were followed up through periodical referrals and hospital visits and/or phone calls. The survival time was considered as the time period from diagnosis to either death or the end of study. The effects of various risk factors such as gender, age at diagnosis, tumor site, pathologic stage, type of treatment, and tumor size were evaluated by Cox Proportional Hazard [PH] model. Data were analyzed using SPSS16 statistical software. Of the total of 400 cases studied, 303 [75.8%] were males. About one third of the patients [130; 32.5%] experienced death. The survival mean and median were 41.8 +/- 3.1 and 26 months, respectively. The percentages of one-, three-, and five-year survivals were 74%, 31% and 23%, respectively. Gender, age at diagnosis, pathologic stage, and tumor size appear to be the most significant factors affecting the survival rate among GC patients. The majority of the risk factors, affecting the survival rate in this study, were related to the delayed diagnosis. Therefore, it seems that if the cancer is diagnosed at early stages, it may lead to better prognosis

ABSTRACT

The Cox proportional-hazards regression and other parametric models model have achieved widespread use in the analysis of time-to-event data with censoring and covariates. However employing Bayesian method has not been widely used or discussed. The aim of this study was to evaluate the prognostic factors in using Bayesian interval censoring analysis. This cohort study was based on 178 patients with gastric cancer from January 2003 to December 2007 admitted to Taleghani teaching hospital in Tehran. Known prognostic risk factors were entered into the analysis using Bayesian Weibull and Exponential models. The term DIC was employed to find best model. The results were showed survival rate depended on age of diagnosis and tumor size. Those patients who had early diagnosis and/or had smaller tumor size were in lower risk of death. The age of diagnosis and tumor size of patients are important prognostic factors related to survival of patients with gastric cancer. Based on DIC, Bayesian analysis of the Weilbull model performed better than the Exponential model. As a result, if this cancer has been diagnosed early, the relative risk of death would reduce
